Follow up
First we'll have a complimentary 20 minute phone consultation to meet each other, determine your needs and select the right services for you.
At this time we will collect Medicare B and any secondary or supplementary insurance carrier information that you or your loved one is enrolled in.
We come right to you to develop a plan customized to your needs.
We offer 2 in-home consultation types to maximize your value and meet your specific challenges.
Telehealth available upon request
Included is a full report with recommendations and resources that fit your needs.
Every assessment includes a follow up phone call consultation to see how the implementations are going and answer any questions that have come up.
Follow-up visits to establish adaptive strategies and home exercise programs, train on home modifications and equipment and provide caregiver training will be scheduled.
