Safe Aging in Place with SAFE-D™
Aging in place safely requires thoughtful planning—to address the full range of risks that can affect independence. SAFE-D™ is the framework developed by Limitless Living Solutions to address the most significant risks to independence as we age. The "SAFE" portion focuses on areas that most often threaten safety at home and in daily life—Slips & Falls, Alerts & Fire Safety, Food & Medications, and Engagement & Connection—giving older adults and their families a clear, proactive plan for safety and confidence at home and on the road.
The "D" refers to driving. Falls can signal driving risk. Older adults who have fallen are ~40% more likely to crash due to shared factors like strength, vision, and medications. Addressing fall risks can also improve driving safety.
Through in-home assessments, practical recommendations, and individualized planning, occupational therapists evaluate how physical, cognitive, and emotional factors affect safety and independence. This includes strength, balance, mobility, vision, confidence, and mood, as well as identifying how anxiety, confusion, or fear may increase everyday risk, enabling clients to age safely where they live.
By focusing on the whole person—and their environment—OTs create personalized SAFE-D™ strategies that may include:

Our Process
Create a safe environment and gain skills to achieve independence within the comfort of your home.
Phone Consultation
We’ll start with a 20-minute phone consultation to get to know you, understand your needs, and identify the right services. During this call, we’ll determine whether your needs meet criteria for medical necessity or are preventative.
We discuss if services will be covered by Medicare Part B and review any potential out-of-pocket costs. If private pay is needed (as in the case of Comprehensive Driving Evaluations and preventative Home Safety Assessments), we will provide a Good Faith Estimate of fees—so you know exactly what to expect before services begin.
In-Home
Assessment
We come to you to develop a customized plan based on your unique needs. Services include in-home Aging-in-Place Assessments and Comprehensive Driving Evaluations to address your specific challenges and maximize value. You’ll receive a detailed report with tailored recommendations and resources to support your safety, independence, and confidence.
Follow up
Every assessment includes a follow-up phone consultation to review progress, answer questions, and ensure recommendations are working effectively.
Additional follow-up visits can be scheduled as needed:

Aging-in-Place: Implement adaptive strategies, home exercise programs, train on home modifications and equipment, and provide caregiver training.

Driving Evaluation: Train on adaptive equipment or strategies, or explore alternative transportation options if driving retirement is recommended.
