
Home Hazard Removal Program (HARP)
Evidence-based fall prevention for older adults in Clay County
HARP is an in-home fall-prevention program led by an occupational therapist. It helps older adults recognize hazards, learn how to prevent falls, and make safe home changes.
In 2026, Limitless Living Solutions offers HARP through a grant from Clay County Senior Services.

Stay Safe at Home—Before a Fall Happens

1 in 4 adults over 65 falls each year, most often in their own homes.

Nearly 90% of older adults want to age in place, but many homes are not set up for safety as needs change.
​​

Preventable falls can lead to injuries, hospitalizations, and loss of independence.
​​

Research shows that fall-prevention programs work significantly better when they consider multiple fall risk factors, not just the home environment, and are led by an occupational therapist (OT).

In the HARP Program, OTs partner with you to look at your abilities, habits, and home, so safety changes are:

How HARP Works
OT + You: A Collaborative Approach​​

ELIGIBILITY
Required:
-
Clay County resident
-
Age 60 or older
-
Enrolled in original Medicare A & B
-
or private health insurance​​​

Reducing your fall risk as your age is important for everyone, but especially if ​you have had recent falls or near-fall or have a moderate to high fall risk*
​

Process
Participants receive:
-
1–3 OT home visits* over 1 month
-
Equipment provided & installed
-
A booster visit after 3–6 months
​
*OT services are billed to health insurance when medically necessary.
Home repairs are paid for by a CCSS grant for eligible participants, according to income guidelines.

Goals
During visits, the OT and client work together to:

Identify fall hazards in the home

Assess abilities, routines, and daily habits

Understand how the home setup and behaviors affect safety

Create a personalized fall-prevention plan

Shower/tub modifications (handheld showers, shower stools/benches)

Improved lighting and motion-detecting nightlights

Smoke and carbon monoxide detectors

Securing or removing rugs

Minor Repairs & Adaptive Equipment:

Removing clutter or furniture hazards

Taping down carpet edges
-
Adding textured or contrasting strips on steps

Adaptive equipment such as bed rails and reachers
