Most people prefer to continue living in their homes as they age. However, without making changes to the home, over time it may become harder to do daily activities such as cooking, bathing, or managing steps. Fortunately, there are simple and affordable ways to change your home to make it work for you.
This is known as Home Modification, making changes to a home to match your changing needs, in order to:

• Support independent living by making daily activities easier
Aging in place requires careful planning, including decisions about changes to the home environment, additional help in the home and keeping the skills that allow you to stay active with safety, mobility and daily activities in mind.

After a thorough home safety assessment, we will offer customized recommendations of equipment and home modifications to reduce fall risk and increase ease of activities. We focus on low-cost, low-barrier, high impact solutions, and help you find resources for their installation.

You will receive an individualized, holistic assessment of your skills, risks and challenges. Together, we will come up with a plan for activities and exercises to reduce your risk of falling and improve your ability to remain independent for the long term.

Since it can be hard to know how to best support your loved one in order to remain safe and independent at home, we include the family and caregiver in our aging in place approach. We have the expertise to train you to best empower your loved one, and offer community resources for you and your loved one.

Follow up
First, we'll have a complimentary 20 minute phone consultation to meet each other, determine your needs and select the right services for you.
At this time we will also collect Medicare B information that you or your loved one is enrolled in.
We come right to you to develop a plan customized to your needs.
We offer 2 in-home consultation types to maximize your value and meet your specific challenges. Included is a full report with recommendations and resources that fit your needs.
Telehealth available upon request.
Every assessment includes a follow up phone call consultation to see how the implementations are going and answer any questions that have come up.
Follow-up visits to establish adaptive strategies and home exercise programs, train on home modifications and equipment, and provide caregiver training, will be scheduled.

Kristen's passion is helping people stay in their homes! Over the years, she has seen the strain on families when illness, surgery or disease makes returning home difficult or impossible. She has witnessed the isolation and lack of support seniors can experience even under Assisted Living care. She has helped countless caregivers navigate their loved ones' advancing needs.
These experiences and passions have lead Kristen to develop an innovative, holistic approach to rehabilitation which centers on what is most important to her clients in their homes. Her goal is to help you make the changes you need, in order to stay safe at home.
