Let's start with a conversation.
We begin with a complimentary 20-minute phone consultation to understand your concerns and goals. During this call, we explain the evaluation process, review provider referral requirements, verify insurance or private pay details, and outline anticipated costs.
After the call, we send secure intake paperwork to gather medical history and background information. Once forms are returned, we schedule your evaluation at a time that works for you — with flexible scheduling options to accommodate work, caregiver availability, and energy levels.

Comprehensive in-home Evaluation
We come to you.
Using evidence-based assessments, we evaluate the person, the home, and the daily activities that matter most — whether that’s fall prevention, home safety, medication routines, or maintaining independence and social connections.
For driving evaluations, clinical testing is completed in the home and the on-road assessment occurs in familiar driving environments in our adapted vehicle to ensure safety.
You’ll receive a clear written report with individualized recommendations, practical strategies, equipment options, and next steps.

Implementation and ONgoing Support
Recommendations are only helpful if they are put into action.
Follow-up visits are scheduled as needed to:

Kristen's passion is helping people stay safe and independent in their homes and on the road. Over the years, she has seen the strain on families when illness, surgery, or disease makes returning home difficult—or when driving skills change, creating new challenges for independence. She has helped countless caregivers navigate their loved ones’ advancing needs with compassion and expertise.
These experiences inspired Kristen to develop an innovative, holistic approach to therapy, focusing on what matters most to her clients: maintaining independence at home and planning for safe driving. By assessing driving skills, preparing for driving retirement when needed, improving home safety, and building practical skills, Kristen helps clients continue to engage in the activities they love. Her goal is to make your dream of aging safely and confidently at home a reality.
